Re: Certifying Signatures Policy and Procedure
POLICY
A certifying signature denotes administrative and financial responsibility in meeting policies and procedures set forth by and for the University of Houston. There are three certifying signatories for the College of Optometry; Dean Jerald Strickland, Associate Dean Roger Boltz and College Business Administrator Mary Juarez. Certifying signatories are responsible for reading and abiding by the various policies and procedures such as Manual of Administrative Policies and Procedures (MAPP), System Administrative Manual (SAM), memorandums, etc.
PROCEDURE
All University financial documents that require a "Certifying Signature" are routed to the Optometry Business Office for documentation process.
All University financial documents that require a "Certifying Signature" must have two certifying signatures. One of the signatures must be the College Business Administrator unless away from campus. Documents must denote such a circumstance for auditing justiication.
Optometry’s internal financial documents must be approved by an internal budget authority.
The internal budget authority cannot "certify" the official University financial document. Example: Dr. Strickland’s documents must be certified by Dr. Boltz and vice versa.
University academic documents that require a "Certifying Signature" are routed to either Dean Strickland or Associate Dean Boltz for signature.
Should there be a change in certifying signatory, a new Signature Card must be signed and forwarded to the Office of Operational Support, Attention Jarry Booth.
This policy will be reviewed yearly and/or revised as needed.
